Health Information

The school can give medicine to pupils when it is prescribed four times a day. Prescribed medicines to be administered three times a day would only be needed if the pupil was staying at school beyond 4pm (attending after school club). For medicines prescribed four times a day, a dose would be needed at school (around 11am) and the third does would only be administered if the child was staying beyond 4pm in after school club.

The form below needs to be completed and returned to the school office with the medicine.  The medicine can be stored in a fridge if needed.
